Creating Meaningful Value In Healthcare

about

Jersey Health Connect is the largest and most comprehensive Health Information Exchange (HIE) in the State of New Jersey (and among the leading HIEs in the nation) providing the infrastructure to electronically connect thousands of healthcare providers and millions of patients.

As healthcare providers move to more efficient models of electronic health record systems, JHC enables NJ providers to exchange vital information with each other through one secure network. The connectivity services provided by JHC are designed to support a spectrum of healthcare providers, from large multi-hospital health systems to solo-physician practices.

A key component of Jersey Health Connect’s success is its ability to connect with disparate certified Electronic Medical Record (EMR) systems. This is in contrast to many health information networks that limit connectivity and services by requiring the use of common software from a specific vendor. JHC’s vendor-neutral architecture enables connectivity with a broad array of health systems, providers, and applications — which elevates the volume and value of patient information exchanged within the JHC network.

As the healthcare industry transitions from a fee-for-service to a value-based reimbursement system, JHC supports population heath management initiatives such as Accountable Care Organizations (ACOs), by utilizing a vast collection of real-time clinical data – to effectively impact healthcare delivery and outcomes. JHC provides electronic notifications to alert providers about important events such as when their patients are admitted to the hospital, receive emergency care at an out-of-town facility, or obtain important test results from referring physicians. Continuity of Care Documents (CCDs) can be securely exchanged between health providers to reduce redundant medical tests and to ensure that patients receive the appropriate care as they transition from one healthcare setting to the next.

JHC currently processes over 2 million electronic transactions per month to help NJ healthcare providers improve operational efficiency and deliver the highest quality, most cost-effective care.
